In order to qualify for the New Practitioner level you must be recently licensed in WA State within the past 90 days.

Background Check and Recurring Payment Information

In order to get signed up we need to run a quick background check. A one-time fee of $50 will be drafted today.
Upon approval and signature of the membership agreement, this card will be used for any monthly dues or room reservations placed in the future and based on the terms of the membership agreement. If you would like to change the card on file, you can replace it anytime with a new active card by giving us a call.

In connection with my membership application with you, I understand that public record information may be requested to evaluate my application.
These reports may include the following types of information: names and dates of previous
employers, public records, credit data, bankruptcy proceedings, eviction and criminal records,
etc., from federal, state and other agencies which maintain such records. I hereby authorize
procurement of these reports. If approved as a member, this authorization shall remain on
